We now provide home and community-based services (HCBS) to persons with disabilities and persons age 18 and older in Hennepin, Anoka, Ramsey, and Dakota Counties.

Basic Support Services including:

• 24-hour emergency assistance
• Adult companion services
• Homemaker
• Night supervision
• Personal support
• Respite care, in home or out-of-home

In-Home Support Services including:

• In-home family support
• Independent living skills training
• Semi-independent living skills

Intervention Support Services including:

• Behavioral support
• Crisis respite, in-home or out-of-home
• Specialist services

We accept the following payment options: Medical Assistance, CADI Waiver, CAC Waiver, Elderly Waiver, BI Waiver, Private Pay, and Private Insurance.
